Connections to Care


The Connections to Care (CTC) program provides comprehensive, coordinated, and evidenced-based services for individuals and families with a serious mental illness (SMI), serious emotional disturbance (SED), or co-occurring disorder (COD) who are also experiencing homelessness and are at imminent risk of homelessness. Services take place where individuals experiencing homelessness spend time—on the street, in the subway system, in a drop-in center, or shelter. This is a uniquely community-based program where 80% of the work takes place right where clients live/spend time. This program focuses on individuals over the age of 55 who have had a difficult time accessing behavioral and primary healthcare.

The goal of the program is to provide effective, equitable, understanding, and respectful quality care in the form of a number of services including, but not limited to:

  • family support

  • housing

  • entitlements

  • employment

  • education

  • vocational interests

  • community integration

  • physical healthcare

  • legal advocacy
